“We've proven that a different model works — one where keeping people healthy actually saves money”Patrick Nelli, Founder and CEO, Aligned Marketplace
What Aligned does
Aligned sells to self-insured employers and third-party administrators a network of independent advanced primary care and direct primary care clinics. The network covers more than 3,000 clinics in all 50 states, within driving distance of more than 80% of the US population, according to MedCity News.
The company ties the fees it takes to member engagement and outcomes, HIT Consultant reported, and contracts directly with practices.
HIT Consultant described Aligned as expanding its direct-contracting model into value-based specialty care while keeping vendor fees tied to engagement and outcomes.
Reported results
Aligned cited a programme it ran for an unnamed Fortune 500 company with a national footprint, launched on 1 January 2025. A third-party actuarial analysis found that engaged members cost 12% less than a risk-matched national benchmark, a saving of $96 per member per month. Members who did not engage with an Aligned doctor came in at benchmark.
Seventy per cent of engaged members were high risk. They averaged 4.7 visits with their Aligned doctor and 1.6 fewer visits elsewhere. After their first visit, engaged members roughly doubled their mammogram rate and nearly tripled their colonoscopy rate. Aligned said 94% of members reported they could get care when needed and rated the experience 9.2 out of 10. These figures come from the company.
The team and investors
Patrick Nelli is founder and chief executive. He said the company has proven that a different model works, one where keeping people healthy actually saves money.
Bob Kocher, a partner at Venrock, said employers continue to struggle with rising healthcare costs and poor access to primary care, and that better primary care lowers healthcare costs by making people healthier. The sources did not give a founding year or headcount.
MedCity News quoted Nelli saying that access alone does not change anything if people do not use it, and Kocher saying Aligned has built a national network of exceptional primary care providers.
What the money is for
Aligned will bring advanced primary care to more employers and third-party administrators, add clinics and build AI tools for scheduling and care navigation, MedCity News reported. It also plans to expand the marketplace into value-based specialty care.
Some aggregators listed the round as $16m; the company's own announcement, MedCity News and HIT Consultant give $20m.
